- First, the following election codes are set up (on the

Codes page):
- One Times My Annual Salary
- Two Times My Annual Salary
- Three Times My Annual Salary
- Four Times My Annual Salary
- Five Times My Annual Salary

The rate table is set up to include premium amounts for one to five times an employee's annual salary:

When you set up benefit elections for the plan, you select Match to Rate Table and select those four election codes on the Benefit Elections - Combined page:

On the Benefit Elections - Match to Rate Table page, you select the corresponding election code for each row in the rate table:
